Integrated Health Care

Integrated Health Care (IHC) is an innovative program of nurse-managed health centers in the Chicago metropolitan area specializing in providing integrated primary and mental healthcare services for people with a serious mental illness (SMI).  Established in 1998, it is affiliated with the Institute for Healthcare Innovation (IHCI) at the University of Illinois at Chicago College of Nursing (UIC CON). This faculty practice has won numerous awards, including the American Academy of Nursing Raise the Voice Edge Runners Award, the National Organization of Nurse Practitioner Faculties Outstanding Faculty Practice Award, and the Illinois Health Connect - Your Healthcare Plus Top Performer “Excellence in Diabetes Management Award” 2011.

IHC clinics are staffed by certified Advanced Practice Registered Nurses (also known as Nurse Practitioners) who work in collaboration with other healthcare professionals to provide primary healthcare, behavioral and mental healthcare services. IHC is committed to the Primary Care Medical Home model (PCMH) and partners with patients and families to best understand and respect each person’s unique needs, culture, values and preferences and to help patients manage and organize their own care. As a PCMH, IHC provides comprehensive, coordinated care and ensures access to healthcare providers 24/7.

In 2007, IHC joined Mile Square Health Center the University of Illinois at Chicago Federally Qualified Health Center, enabling IHC to expand its scope of services to all community residents in addition to our focus population.  IHC’s partners assist with the systems and support an efficient and effective Academic Nursing Center. In 2010, IHC was one of only 10 Nurse Managed Clinics in the U.S. to receive funding from President Obama’s Affordable Care Act.
